Job Description

St. Patrick Center provides opportunities for self-sufficiency and dignity to people who are homeless or at risk of becoming homeless. Individuals and families build permanent, positive change in their lives through safe and affordable housing, sound mental and physical health, and employment and financial stability. 

Working collaboratively with individuals, businesses, government and other service providers, St. Patrick Center will end chronic homelessness in the St. Louis region through Housing First, an evidence-based practice that tells us to remove barriers, do intake and assessment early, move people into housing and offer them support to achieve their goals. 

Job Summary

St. Patrick Center, a ministry of Catholic Charities, is hiring a Mental Health Specialist as part of the Assertive Community Treatment (ACT) team. This specialist will provide treatment, rehabilitation, advocacy, and case management services to program clients with serious, persistent mental illnesses and co-occurring substance use disorders. This specialist will also provide a-centered approach to direct clients to resources. As part of the ACT team, the specialist will act as an advocate and liaison while consulting with community agencies and program clients. The core working hours of this position are Monday to Friday 8:00 am - 4:30 pm. As a benefit for our employees, we offer free supervision and student loan forgiveness opportunities.  

Job Responsibilities

• Conduct comprehensive, ongoing assessments of program clients’ mental illness, physical health, housing status, and other relevant needs
• Manage primary case load by completing treatment plans, DLA-20s, and coordinating care through monthly client schedules
• Provide person-centered direct clinical services to clients on an individual and group basis in the office and in the community settings, making appropriate changes in treatment plans to ensure clients’ ability to cope with internal and external stressors
• Facilitate clients’ treatment planning with an emphasis on fostering personal growth and development towards recovery and wellness
• Refer clients to appropriate community agencies, assisting clients when needed
• Act as advocate and liaison, consulting with community agencies and program clients to maintain coordination in the treatment process
• Collaborate with other members of the ACT team during daily and weekly staff meetings
• Provide on-call crisis intervention in coordination with other ACT team members, covering nighttime hours, holidays and weekends
• Maintain St. Patrick Center (SPC) client tracking system and client files (including treatment plans), documenting client progress to maintain a permanent record of client activity within the timeframe required by DMH and BJC/administrative agent (24 hours)
• Document all interventions within 24 hours into My Avatar
• Responsible for agency policy regarding work hours, building security and all written personnel standards
• Ensure the care model by interacting with all other SPC programs
• Attend and participate in all appropriate SPC meetings
• Other duties as assigned

 

Job Requirements

  • Master’s degree in counseling, social work, or a related field from an accredited university required
  • Qualified Mental Health Professional Required
  • Driver license required (MO residents require Class E)
  • Current and valid auto insurance required
  • Reliable vehicle required
  • Ability to work a minimum of 37.5 hours per week
  • Experience in counseling and case management/wrap-around services
  • Experience in conducting home visits
  • Knowledge and understanding of community agencies
  • Knowledge of federal, state and agency HIPAA policies
