Essential Duties:

• Accept, review, and follow up on referrals from law enforcement agencies received on the 800 lines for clients in need of SUD services
• Conduct “warm hand off” by escorting clients to designated treatment locations or arranging their transportation.
• Supervise case managers to ensure clients are engaged with the treatment team regarding patient attendance, participation, and any barriers or needs to support treatment involvement.
• Distribute Harm Reduction Materials: Deliver harm reduction supplies and resources to individuals within specified zip codes, including syringes, condoms, naloxone, and educational materials.
• Community Engagement: Establish rapport and trust with community members while providing information on harm reduction strategies, substance use disorder treatment options, and access to support services.
• Outreach and Education: Conduct outreach activities such as street-based outreach, community events, and educational workshops to raise awareness about harm reduction principles and promote safer practices.
• Collaborate with Partners: Collaborate with local organizations, healthcare providers, law enforcement, and community leaders to coordinate outreach efforts and facilitate access to resources for individuals in need.

Work Schedule: Monday - Friday (7:00a - 3:30p)

Pay Rate: $20.00

Experience and Skills:

Education and/or Experience: High School Diploma / GED required; Experience in/with substance use, detox programs, medication-assisted treatment, behavioral healthcare and/or community relations encouraged to apply .
